What to Expect with Your Fellow Kits
& Post Op Semen Analysis
Step 1:
You will scan the QR code in your discharge book or utilize the link below to complete your order & get $20 off per kit when purchased separately. You are responsible for purchasing your kit directly from Fellow.
Please, select your surgeon/provider, Dr. Sheldon Marks or Dr. Peter Burrows, when purchasing your kit. Your results will NOT be uploaded into our system if you do not select your surgeon from the drop down.
Step 2:
Fellow Ships to You
Fellow will ship your kit directly to you once you have completed your order. Your kit will also come with a pre-paid UPS shipping label inside of it for after your collection.
Step 3:
Collection Instructions
- Produce your sample on a Monday, Tuesday, Wednesday, or Thursday morning
- You will need to abstain from ejaculating for at least 48 hours, but no more than 5 days, when you produce your sample
- When ready to produce your sample, please open your kit and follow the instructions provided inside.
- Sample Registration is REQUIRED AT THE TIME YOUR PRODUCE YOUR SAMPLE, using the QR code on the inside of your kit. Without this information, Fellow cannot calculate your results.
- Cryopreservation is available as an add-on service for an additional cost; to utilize this service, you must opt-in when you register your kit.
- Sample must be dropped off at UPS the same day as collection; your kit comes with a pre-paid UPS shipping label.
Step 4:
Your Test Results
Once Fellow has received your sample, your results will be available to you within 2 business days, and then will be uploaded directly to your provider (us).
Please, allow 3-5 business days for our office to email you your results + your surgeons recommendations going forward, based off your results.
