Your Vasectomy Reversal Onboarding Experience at ICVR
Step 1:
Deposit for Surgery Date - Due at Booking
We require a $1500, non-refundable deposit to secure your surgery date on the surgeons calendar. Upon booking your date, you will be sent a text message as well as an email from our finance department with a link that allows you to pay online! You are welcome to pay your surgery balance in full at the time of booking to 100% secure your date on the surgeon's calendar.
If we do not receive your deposit within 24 hours of booking your surgery date, you will be sent a notification and removed from the surgery calendar.
*Please note, if you have any issues with your payment we will be notified in our system and reach out to you directly.
Step 2:
Surgery Balance Payment - Due 6 Weeks Before Surgery Date
Six weeks before your surgery date, you are required to pay your surgery balance to finalize your date on the surgery calendar. If you cannot pay your surgery balance 6 weeks before your surgery date, you will be removed from the calendar and your deposit will not be refunded.
- If you are paying in full or utilizing a third-party lender, the remainder of your surgery balance will be due in full at this time.
- If you are a PVPS patient, your surgery balance is due in full at this time.
- If you are doing our in-house financing, the remainder of the 70% up-front cost will be due at this time.
Step 3:
Introduction to the Fellow Kits & Pre-Operation Paperwork
We know the dread of more paperwork, but fear not - it's all digital, even the semen analysis kits!
We offer 6 months of semen analysis review from your surgeon included in your surgery price when you utilize our partnership with Fellow, an at-home semen analysis kit! See some of the perks below:
- $20 off per kit when bought every 4 weeks with our custom ICVR QR code
- Kit are delivered directly to your door!
- At-Home Collection Process, no more labs
- Results within 2-3 days of being received by the Fellow lab
- We get your results directly in our portal within two days of when you get your results, no fax machines or office communications required!
Pre-Op Paperwork
Our office will reach out to explain the required pre-op paperwork that will be sent over via Docusign. You will be required to digitally sign three different documents:
- Surgery Agreement
- HIPAA Agreement
- Freezing Disposition
Step 4:
Travel Accommodations
We have partnerships with two different hotels located closely to our office in Tucson that will offer you better rates by using our links below. We strongly encourage you to book with these hotels, as they will put you in rooms closest to the elevators as well as the ice machines, and they are within close proximity of our office. Staying at one of these also insures that your surgeon will be able to do your "house call" the morning after your surgery, allowing you to ask all those questions that come up the first night after surgery as well as have a full post op-exam from your doctor.
Step 5:
Uploading Identification
You will need to Upload the Following Photos:
- Front of Drivers License / State ID / Passport (Military IDs are NOT accepted)
- Back of Drivers License / State ID / Passport (Military IDs are NOT accepted)
Step 6:
REQUIRED Pre Op Instructions Call(s) with Your Nurse
Two weeks before your surgery date AND the day before your surgery date, you are required to complete a pre op call with a nurse from our office. Your spouse or significant other CANNOT do your pre op instructions calls for you.
- Please, allow 20-25 minutes for the initial pre-op instructions call that is two weeks before your surgery date (see your onboarding email for the exact date provided to you)
- In this call, you will go over your entire medical history, current medications and supplements, as well as what to expect the day before and morning of your surgery.
- After this call, you will then be provided a PDF document of the exact instructions for the two weeks leading up to your surgery date and additional information about your post operative care as well.
- You will also be called the day before your surgery date to go over all the details of your surgery, your stay in Tucson, travel accommodations and also answer any questions you may have before your big day!
